Western Communities Primary Care Network

The Western Communities Primary Care Network acknowledges and gives thanks to the Lkwungen peoples, also known as the Songhees and Esquimalt First Nation communities, for allowing us to live, work, and play on their lands. We also respectfully recognize other surrounding nations Scia’new (Beecher Bay) First Nation, T’Sou-ke (Sooke) Nation, and Pacheedaht First Nation (Port Renfrew).

Overview

A Primary Care Network (PCN) is a community-based network of health care professionals and clinics who plan and deliver the health care needs of a community.

The Western Communities Primary Care Network (PCN) team is working with local clinics to help connect people to a primary care provider, either a family doctor or nurse practitioner. This includes people living in the following communities:

  • Colwood
  • Langford
  • Highlands
  • Esquimalt
  • View Royal
  • Metchosin
  • Sooke
  • Juan de Fuca Coast

If you need a primary care provider, register on the Health Connect Registry to get matched with a family doctor or nurse practitioner in your community.

What's happening in your community

Call HealthLink BC at 8-1-1 to speak with a health service navigator, registered nurse, registered dietitian, qualified exercise professional or pharmacist. You can contact HealthLink BC 24 hours a day, 7 days a week.

Pharmacists are an important part of your health care team and offer a variety of services. They are able to assess and prescribe for 21 minor ailments, prescribe contraceptives, renew prescriptions, and give vaccines.
