The Surrey North Delta Primary Care Network provides its services within the traditional, ancestral and unceded shared territories of the q̓ic̓əy̓ (Katzie) [kut-zee], q̓ʷɑ:n̓ƛ ̓ən̓ [Kwantlen], kʷikʷəƛ̓əm (Kwikwetlem) [kwee-kwet- lum], Semiahmoo [semi-ah-moo] and sc̓əwaθən məsteyəxʷ (Tsawwassen) [tsawah-sen] First Nations and home to the Surrey-Delta Métis Association.
A Primary Care Network (PCN) is a community-based network of health care professionals and clinics who plan and deliver the health care needs of a community.
The Surrey-North Delta Primary Care Network (PCN) makes it easier for Family Doctors and Nurse Practitioners to provide the best possible care for their patients. Centering on the needs of patients and their families, our PCN connects primary care providers, health care services and community services ensuring that patients and their families in our community can access the health supports they need. The Surrey North Delta Primary Care Network attachment team is actively matching patients with family doctors and nurse practitioners in the community. If you need a primary care provider, register on the Health Connect Registry to get matched with a family doctor or nurse practitioner in your community.
When a family doctor or nurse practitioner in your area becomes available, a staff member from a medical office or clinic in your community will contact you either directly or by email. Add 'attachment@snddivision.ca' as a trusted sender in your email and check regularly for updates.
To ensure you can be reached, keep your registration information up to date at https://hcr.healthlinkbc.ca/s/patient-updates or by calling 8-1-1. You can update your address, phone number and email address, share new and recent health changes, and confirm you still require the services of the Health Connect Registry.
