Surrey-North Delta Primary Care Network connects communities with family doctors, nurse practitioners and health services for accessible, community-based care.

The Surrey North Delta Primary Care Network provides its services within the traditional, ancestral and unceded shared territories of the q̓ic̓əy̓ (Katzie) [kut-zee], q̓ʷɑ:n̓ƛ ̓ən̓ [Kwantlen], kʷikʷəƛ̓əm (Kwikwetlem) [kwee-kwet- lum], Semiahmoo [semi-ah-moo] and sc̓əwaθən məsteyəxʷ (Tsawwassen) [tsawah-sen] First Nations and home to the Surrey-Delta Métis Association.

Overview

A Primary Care Network (PCN) is a community-based network of health care professionals and clinics who plan and deliver the health care needs of a community.

The Surrey-North Delta Primary Care Network (PCN) makes it easier for Family Doctors and Nurse Practitioners to provide the best possible care for their patients. Centering on the needs of patients and their families, our PCN connects primary care providers, health care services and community services ensuring that patients and their families in our community can access the health supports they need. This includes people living in and around:

  • Surrey
  • North Delta- West Newton
  • North Surrey-Whalley
  • Guildford
  • Cloverdale-Panorama
  • Fleetwood
  • East Newton

If you need a primary care provider, register on the Health Connect Registry to get matched with a family doctor or nurse practitioner in your community.

When a family doctor or nurse practitioner in your area becomes available, the team in your community will contact you. They may also contact you by phone or email for more information or to offer health care support while you wait to be matched.

The Surrey North Delta Primary Care Network attachment team is actively matching patients with family doctors and nurse practitioners in the community. The team understands the urgency to find a provider and is moving as quickly as possible.
