The Burnaby Primary Care Network provides its services within the traditional, ancestral and unceded shared territories of the xʷməθkʷəy̓ əm (Musqueam) [mus-kwee-um], Sḵwx̱wú7mesh (Squamish) [Squ-HO-omeesh], and səlilwətaɬ (Tsleil-Waututh) [slay-wha-tooth] First Nations and home to the North Fraser Métis Association.

A Primary Care Network (PCN) is a community-based network of health care professionals and clinics who plan and deliver the health care needs of a community.
The Burnaby Primary Care Network team is working with local clinics to help connect people to a primary care provider, either a family doctor or nurse practitioner. This includes people living in the following communities:
- Burnaby
- Lougheed
- Brentwood/Hastings
- Edmonds
- Metrotown

Foundry Burnaby has secured a site in South Burnaby. Foundry Burnaby will operate at 6889 Royal Oak Avenue, neighbouring Royal Oak Skytrain Station and a developing urban area close to Metrotown.
