A Primary Care Network (PCN) is a community-based network of health care professionals and clinics who plan and deliver the health care needs of a community.
Through team-based care, patients in the East Kootenay PCN (EKPCN) can access a broader range of health professionals working alongside their family physician or nurse practitioner to support their ongoing health and well-being.
EKPCN is working to improve the health of the population through timely access to comprehensive, person-centred, team-based care that is equitable and culturally safe. Teams may include family physicians, nurse practitioners, registered nurses, allied health professionals, clinic staff, midwives, Aboriginal Care Navigators, and other health-care professionals who collaborate to provide coordinated care. The East Kootenay Primary Care Network is working with family practice clinics in East Kootenay communities to help individuals and families find a family doctor or nurse practitioner. Most clinics strive to accommodate urgent needs, but appointments are not always available. If you are unable to see your own family physician or nurse practitioner, or do not have one, visit an Urgent and Primary Care Centre in the East Kootenay:
Pharmacists are an important part of your health care team and offer a variety of services. They are able to assess and prescribe for 21 minor ailments, prescribe contraceptives, renew prescriptions, and give vaccines.
