The Ministry of Health has developed the HealthyBC Self-Assessment Tool in collaboration with Thrive Health. You are invited to use the tool if you are aged 50 or older. The tool is a questionnaire on selected common health topics of interest, and can help you:

  • Self-reflect on common aspects of health
  • Navigate and access the healthcare system
  • Learn ways to maintain independence and control over your health and wellness
  • Get access to online resources to support your health as you age

About the Assessment Tool

The Assessment Tool has approximately 45 multiple-choice questions and will take you about 20 minutes to complete. The questions cover a range of topics, including: 

  • B.C. health system navigation and benefits access
  • Preventive Health Screenings
  • Emotional Well-being  
  • Physical Well-being
  • Chronic Disease Prevention
  • Sexual Well-being
  • Social Well-being
  • Emergency Preparedness

Your survey results

After completing the questions, you'll immediately receive a personalized health summary report, which you can print or save for your records.

This report includes recommendations for wellness resources and guidance to help you navigate the health system.

You’ll also have access to a library of recommended resources that you can explore at your own pace.

Please note: The Assessment Tool is not intended for clinical diagnosis, and your responses will not be reviewed by a healthcare professional.

Optional account creation

You will also have the option to create a Thrive Health account that offers enhanced Assessment Tool features, including:

  • Saving your results for access to your personalized recommendations at any time
  • Access to the recommended resource library over time
  • A trends feature that allows you to view changes to your health domain scores as you take the self-assessment more than once 

I need help

If you need help using the tool, you can also ask a member of your family or someone you trust with your health information to complete it for you.
