By registering for the Health Connect Registry, the attachment coordination team in your community is made aware that you need a family doctor or nurse practitioner.

What happens next
The attachment coordination team in your community will review your registration details. We may contact you for more information, to provide updates on your registration, or to offer you information on health services available while you wait to be matched.
Some people have serious health conditions that need timely assessment and treatment. People who need care the most are prioritized and this may impact how long you wait.
When a family doctor or nurse practitioner in your area becomes available the team will contact you to complete the intake process. This usually includes an appointment with the family doctor or nurse practitioner so you can meet. If you both agree, you will become “attached” to the family doctor or nurse practitioner as your long-term primary care provider.
If the match is unsuccessful, you will remain on the Health Connect Registry and we will find you another match. You will maintain your place on the Registry.
Make a change to your registration
Keeping your registration information up to date will help us make the best match. Call 8-1-1 to update your registration information. You can:
- Add or remove a family member
- Update your address or contact information
- Update us on any new health changes in the last three months
- Inform us if you’re no longer looking for a family doctor or nurse practitioner
Find local health services and supports
We recognize you may have been waiting for some time to find a family doctor or nurse practitioner. While we are unable to estimate how long it will take to be matched, health care services are available while you wait. You can access health care services in your community by visiting an Urgent and Primary Care Centre, Community Health Centre or First Nations Primary Care Centre, a walk-in clinic, or by accessing services through your local pharmacy. You can also call 8-1-1 to speak to a variety of healthcare professionals including registered nurses, pharmacists and mental health professionals.
