Provides medical care, education and counselling to help slow or stop the loss of kidney function and promote wellness for patients and families affected by chronic kidney disease. Clinics when required, may also prepare patients for kidney dialysis. The program also provides follow up care, monitoring and support for renal transplant recipients.
