Offers information and personalized support to help connect older adults in the community to non-clinical, community-based services that reduce isolation, promote independence, and help to improve overall health and wellness.

Community connectors help seniors with accessing services such as social and physical activities, food programs, caregiver supports, cultural programs, legal and housing services, transportation, and mental health services.

Eligibility
Older adults living in the Surrey area, aged 60 and over, who are marginalized, experiencing poor mental and or physical health, socially vulnerable, or making frequent trips to the emergency department or primary care clinic.
