Pediatric Nursing Support Services

Provides community-based nursing support, case planning and health assessments for children and youth with complex medical conditions such as diabetes, seizures, tube feeding, and catheters. The program includes individualized care plans, caregiver training and education, health status monitoring, and contact with school-based teams, as needed.
