Supports individuals who have a life-threatening illness or are facing end of life issues, and who wish to receive palliative care at home, hospice, residential care facility, or hospital.

Care is provided to patients and families by a team who provide physical and emotional support, information on services and care options, teach families how to provide or assist with care, and referrals to available services such as volunteers or counselling.
