Medication Management Program

Offers in-home medication review and management to older adults at risk of medication misadventures, including those recently discharged from hospital taking six or more regular medications. A pharmacist reviews all medication including outdated and unneeded medication, and makes recommendations to the patient and or family doctor.

Eligibility
Older adults aged 65 and over, taking more than six medications, at risk of drug interaction, with kidney or liver failure, or with dementia or confusion. Nursing home clients are excluded from this service.
Application process
Hospital care team, physician, and home health care staff referrals accepted.
