Provides a community-centered health promotion program that supports Chinese, South Asian, and Aboriginal communities, patients and caregivers across BC to optimize chronic disease prevention and self-management. Health information and resources are provided online and as well, through outreach skills-building workshops, public health forums, and through media projects such as community television programs.
