Provides in-home support services to those with chronic health conditions, disabilities or terminal illness so they can continue to live at home safely and independently.

Trained community health workers offer assistance with daily living activities such as bathing, dressing and grooming, as well as provide special exercises; caregiver rest and relief, and medication administration.
