Provides in-home nutrition assessment, counselling and education to individuals who are not mobile enough to reach a clinic. Dietitians help clients to manage health issues including diabetes, swallowing disorders, tube feedings, chronic health conditions, and palliative care.

Eligibility
Adults aged 19 and over who are unable to leave their home. Clients who wish and are able to be seen in their community health centre may be referred to an ambulatory home health dietitian. Clients must meet eligibility requirements to receive services.

Call the Home Care Access Line for more information or to request service. Current Home Health clients should contact their local office for questions regarding their enrolled services.
