Eligibility
Adults aged 19 and over released from hospital and who need short-term care; or have an ongoing chronic health issue needing care; or have a health issue that is getting worse and support is needed to continue living at home.
Application process
Call the Home Health office in your area for intake. Current Home Health clients should contact their case manager for questions regarding their enrolled services.
