Home and Community Care Access Line - Vancouver Coastal
Provides intake for home health and community care services for residents with acute, chronic, palliative or rehabilitative health care needs. Case managers provide assessments, evaluate the client needs, determine eligibility and provide recommendations for services to be offered.
Services include case management, home care nursing, community occupational and physical therapy, home support, community nutrition, hospice palliative care, adult day programs, caregiver support, convalescent care, assisted living and residential care.
