Home and Community Care
Offers a range of at-home and community services to eligible residents with acute, chronic, palliative or rehabilitative health care needs. Case managers provide assessments, evaluate the client needs, determine eligibility and provide recommendations for services to be offered.
Services include assessment and case management, nursing, rehabilitation, caregiver support, home respiratory services, support with activities of daily living, palliative or hospice care, and community nutrition.
