Provides education and remote monitoring to individuals with chronic pulmonary disease (COPD), chronic kidney disease (CKD), diabetes, heart failure, hypertension, influenza, positive and symptomatic COVID-19, heat-related illness, smoke inhalation, or life-limiting and palliative illness. Participants answer a few questions and do daily measurements such as blood pressure, weight or oxygen level.
Results and responses are sent to a registered nurse who looks for changes in measurement. The nurse is in regular contact with patients to review results, and communicates concerns to the family physician or nurse practitioner. The program also includes support for caregivers to build coping skills through one-on-one or group counselling offered online.
