Community Rehabilitation
Provides in-home assessments, consultations, treatment, and education to eligible clients and their families. Therapists help clients restore, improve or maintain physical capabilities and ensure the home environment is organized to make daily tasks more manageable.
Services include training in the use of mobility devices like walkers and crutches, home modification recommendations, and teaching exercises to improve muscle strength, balance, and coordination.

Eligibility
Clients must meet standard home health care eligibility requirements and require care following discharge from a hospital, care at home rather than hospitalization, or care because of a terminal illness.

Referral required. Eligibility must be assessed by a Home Health case manager; contact your local Home and Community Care office for more information.
