Bridging Care Program

Provides a short-term intensive day treatment for people with acute and severe mental health challenges. Participants attend daily group sessions, participate in discussions and skills training, and receive one-on-one counselling to practice and reinforce skills that are relevant to their goals. Before discharge, participants have a plan in place to continue to support their wellness and goals, and may be connected to further supports in the community.
