Breathe Well Program
Provides intensive, home-based respiratory care, education, and case management to help patients discharged from the hospital with managing their Chronic Obstructive Pulmonary Disease (COPD).
Services include inhaled medication education and review, cardio respiratory home assessments, ongoing education and home-based activity programs, COPD flare-up plans, and tobacco dependency counselling.

Eligibility
Individuals with moderate to severe COPD, who have required 2 emergency visits and or a hospital admission for their COPD in the past 12 months.
Application process
Referral required by family physician, or any member of the person's Interior Health care team, or by calling the nearest IHA Home Health office.
