Provides assessment, education and treatment to patients with abnormal heart rhythm, called an arrhythmia. Patients meet with a team of health care providers including nurse practitioner, electrocardiography physician and technicians, clinical resource nurse, and pharmacist. Patients are also provided with education sessions, and telephone follow-up with members of the care team.
