ACT Team
Provides community-based support for adults with persistent mental health issues that make it difficult to manage their daily living. Program aims to improve quality of life and decrease time spent in hospital and with emergency services.

Support includes accessing medical care, substance use counselling and or access to treatment, life skills support, medication management, social activities, and connecting to community resources.
