Adult Day Program
Provides a range of health, social and therapeutic recreational day programs to help improve and maintain the well-being of older adults and adults with disabilities in the community. Services are designed to help individuals to remain at home and may include health education and monitoring, personal care, bathing programs, therapeutic social programs, and recreational programs.

Case manager referral and assessment required. Contact your case manager or local Home Health office for more information; do not contact the facility directly.
Program fee
A small daily fee will be charged to cover costs of transportation, meals and supplies. Please call for additional information.
