Provides a range of health services to help Trail residents meet their everyday health care needs in order to stay healthy, prevent illness, and manage ongoing illness or disease.

Eligibility
Individuals and families living in the Lower Columbia region who do not have a family doctor and have trouble accessing health care services in their community.
Application process
Patients must be registered and attached to the clinic before receiving services; no walk-in services offered. Call before faxing confidential patient information.
