SAIL
Offers a fall prevention program designed to promote the safety and independence of older adults living in the community and receiving home support services. The home health case manager, community health worker, and the client work together to ensure a safe living environment in the home. Following an assessment conducted by home health, community health workers visit eligible clients in their home to assess the need for changes required to improve safety.
