Provides assistance to parents and caregivers of children with special healthcare needs to lead active, healthy lives in their communities. Support may be provided in home, school or childcare settings. Services include, but are not limited to, planning of community care, information and consultation about health issues relating to the special need, at home assessments, and training of alternate caregivers.
