Provides team-based primary care services for Island residents who do not have a regular primary care provider. Patients are registered to a nurse practitioner and will have access to regular and follow-up visits.

Services include diagnosing and treating illness, chronic disease management, prescription refills, mental health and addiction support, and referrals to medical specialist and social services.
