Provides multi-disciplinary outpatient assessment and rehabilitation for adults with a neurological condition such as stroke, head injury, multiple sclerosis and Parkinson's. Priority is given to clients being discharged from the hospital.

The program helps patients improve their physical, cognitive and social functioning of neurological conditions through individual and group treatment.
