Provides intake for home health and community care services for residents with acute, chronic, palliative or rehabilitative health care needs. Case managers provide assessments, evaluate the client needs, determine eligibility and provide recommendations for services to be offered.

Services include assisted living and residential care, home support, personal care, respite, fall and injury prevention, palliative or hospice care, nutrition services, and adult day programs.
