Happy Hearts Alliance
Offers cardiac wellness programs to support individuals with heart disease or at risk for heart disease. Provided in partnership with VGH Centre for Cardiovascular Health, St Paul's Hospital's Healthy Heart program and community organizations.
Provides cardiologist and case manager support, exercise classes, nutrition and psychosocial support, weekly education sessions, and referrals to community resources. Also offers community-based support for people who have graduated from a cardiac rehabilitation program by trained community cardiac fitness instructors. Programs may vary by location.
