Choosing a Substitute Decision-Maker (SDM)

British Columbia Specific Information
On September 1, 2011, advance directives became a new legal option for capable adults in British Columbia who want to do advance care planning to make their wishes known for their future health care treatment decisions. Advance care planning is the process of thinking about and writing down your wishes or instructions for future health care treatment in the event you become incapable of deciding for yourself.

The Ministry of Health encourages all capable adults in B.C. to do advance care planning, and take the further step of documenting their wishes and instructions for future health care using the tools available in the advance care planning guide, My Voice: Expressing My Wishes for Future Health Care Treatment (PDF 4.29M).

More information on advance care planning is found on the Ministry of Health website on Advance Care Planning. You may also email AdvanceCarePlanningBC@gov.bc.ca if you have questions.
